The Reporter Preference Form will be our go-to document to use style and preferences while working on your transcript.
Click on the link below, when asked to save, choose “save as” and add your first name to the document name. You can save the form in your Dropbox folder so we have access to it while working on your transcripts.
You are not required to fill out the form in full. Fill out as much or as little as you like. The form is lengthy as we tried to cover as many scenarios as we could, and I’m sure there are things we missed.
Since the form will be in your Dropbox folder, you can work on it or update it whenever you like.
If something comes up in your transcript that is not covered by the information in the form, we will contact you for your preference.
